Important information before you start

This form is for submitting claims for an order that was lost, delivered but not received, damaged, stolen after delivery, or where items are missing. You'll need store name and order number to locate your purchase.

One issue per submission

Submit one order/shipment issue per claim. If multiple items were affected by the same delivery issue, include them in one submission.

Have these ready (if applicable)

  • Order number (from your confirmation email)
  • Tracking link/number (or a screenshot)
  • Photos (if your item arrived damaged)

To avoid delays

Upload any relevant screenshots/photos during submission. If anything is missing, we'll email you with a clear list of what we need next.
